All Grades Will Need

• Birth certificate
• Updated immunizations
  • CDPH Immunization Requirements K-12 (English | Espanol)
• Proof of address*

If residing with someone- please provide the following Co-Residency Verification Affidavit (English | Espanol)

Transitional Kindergarten/Kindergarten

• School entry physical
• Oral health exam.

*Make the Doctor's appointment for the physical as early as possible. These appointments are harder to get closer to the start of school.

7th Grade

• Tdap booster
   

Highschool

• Current transcripts
   

Health Conditions/Disabilities

• Report of Health Examination for School Entry (English | Espanol)
   

Process

Gather Required Documents
